Summer Camp Registration

I hereby authorize and agree that Faith Christian Academy (FCA) has the right to capture and use photographs of my child and to use these photographs for the purpose of school publications such as, but not limited to, the school newspaper, yearbook, principal's newsletter, photo albums, and more now and hereafter known, and for the purpose of local newspaper articles/features, fundraising brochures, school profiles, and more, and for the purpose of the school website (www.fcagoldsboro.com). I hereby release to FCA all rights to exhibit this work in print and electronic form publicly or privately and to market copies. I waive any rights, claims, or interest I may have. I understand and agree that any uses herein may be made without compensation or additional consideration.
